The application form (Request) and the form to request preliminary examination (Demand) are available on the WIPO websiteOpen link in a new tab.

• The application form (Request) has the number PCT / RO / 101
• The form to request preliminary examination (Demand) has the number PCT / IPEA / 401.

There are instructions at the end of the forms.

You can fill in the form online in "Editable pdf" format.

Use the "Filled-in sample of the Request Form" and "Filled-in sample of the Demand form" as examples when you fill in the forms.
